Eligibility

To access community nursing services through the DVA Community Nursing Program, you must Hold a Gold or White DVA Card

We accept referrals from:

• General practitioner (GP)
• Treating medical practitioner in a hospital
• Hospital discharge planner, or
• Nurse practitioner specialising in a community nursing field
